Sometimes individuals need specialized services following a hospital stay.

Paramount Community Living And Rehab’s Health Care Center and Green House homes are Medicare-certified for providing these services, such as physical, occupational and speech therapy and skilled nursing.

The ultimate goal is to have each individual attain and maintain as much independence as is realistically possible through a personalized program of treatments and activities.

All nursing rooms in both Nelson Hall and the Green House homes are private.

Alzheimer’s Care

Our nurses work with the elder’s family to create a comfortable environment including simple routines and visual cues as a positive substitute for diminishing mental processes.

Diabetes Management

Our experienced nurses and Registered Dietitian provide help with all aspects of managing diabetes including: blood glucose monitoring, insulin administration and tracking, exercise plan, menu planning, education and more.

Restorative Nursing Program

At Paramount Community Living And Rehab we seek to maximize the quality of life for our elders, who require a specialized restorative program. The therapy team and nursing staff develop individualized treatment plans for resident care in the areas of

Restaurant style dining served three times a day
